An "Authorization for the Administration of Medication by Camp Personnel" form needs to be returned only if the camp is to dispense medication to your child.  Both the parent and doctor must sign this form.

Medication to be dispensed by the camp nurse must be in the prescription bottle and must be accompanied by a doctor's order.
